How to Make an Appointment

To begin the appointment process, please complete a New Patient Registration Packet. This packet may be printed from our website or picked up in person at one of our offices located at:

Once you have fully completed the New Patient Registration Packet, please bring the packet to our Medical Office Front Desk with your ID and insurance cards. After processing is complete, our Call Center will contact you to schedule your first appointment.

What to Bring to Your Appointment

To ensure that you have everything you need during your appointment, be sure to bring the following with you:

• Insurance Cards – Please be sure to bring your insurance card with you for all your appointments. If you have secondary insurance, please bring the secondary insurance card as well.
• Medications – If you are taking over-the-counter or prescription medications, supplements, or vitamins, please bring them in their original containers.
• Allergy List – If you have allergies, please make a list to show your provider.
• ID – Please bring a valid form of identification.

Please Arrive Early to Your Appointment

We encourage patients to arrive 10 minutes prior to their scheduled appointment time to complete any updates or address any information that may have been missed.

Types of Insurance Accepted

Lassen Indian Health Center accepts most types of health insurance, including Medi-Cal, Medicare, and Covered California plans from Anthem Blue Cross and Blue Shield. Patients with private PPO coverage are also welcome. If you have questions regarding coverage, please call our office prior to your appointment.

Co-Pays & Accepted Payment Methods

We accept cash, personal checks, and most major credit cards. Please note that all co pays are collected at the time of your appointment.
