At Lassen Indian Health Center, we are pleased to offer our non-emergency Transportation Service to eligible patients. Our Transportation Service gives patients the opportunity to get to their scheduled appointment simply by scheduling a ride. Apply today to see if you are eligible.


LIHC Transportation Service

Take advantage of our transportation service! Follow the steps below to check for eligibility, complete the application and schedule transportation to your next LIHC appointment.


Lassen Indian Health Center Transportation Services are available to patients who do not have access to other forms of transportation. Most insurance companies provide transportation services. If you have insurance but are ineligible for or unable to use their transportation service, please attach supporting documentation when you submit your application. When applying, patients may need to demonstrate the following:

  • No other means of transportation
  • Medical condition that makes driving difficult

How to Apply:

If you are a current patient at Lassen Indian Health Center and meet one of the criteria above, you are eligible to apply for our Transportation Services. Simply download and complete the “Transportation Application” listed below.

How to schedule your transportation:

Scheduling your transportation service is easy. Once you have filled out your Transportation Application and have been accepted, simply call the transportation scheduling office to set up your transportation service appointment. The scheduling office is open Monday through Friday from 8:00AM to 5:00PM. Just be sure to schedule your transportation service with at least 48 hours’ notice.

When you are ready to call the transportation scheduling office, be sure to have the following information ready:

  • Name
  • Date of Birth
  • Home Address
  • Phone Number
  • Your LIHC Appointment Information including Date of Appointment, Time of Appointment, and the Name of the Provider you are seeing.
  • Pick-Up Location Address
  • Type of Service(s) needed such as wheelchair accessibility, restraint, parent/guardian on vehicle, curbside pick-up, or other

Frequently Asked Questions

Below are some answers to the most common questions about non-emergency transportation services at Lassen Indian Health Center. If you have futher questions, please give us a call and we would be happy to help.

How early should I be ready for my ride?

Please be ready and waiting at least 15 minutes before your pickup time. If you are not present at your pickup location, you may miss your ride.

Do I have to apply to take advantage of LIHC Transportation Services?

Yes. All current LIHC patients are eligible to apply for Transportation Services. The Transportation Services Application can be downloaded for your convenience.

May I schedule a ride for my child if they are under the age of 18?

If your child’s application is approved they will be able to use the Transportation Service; however, they must be accompanied by a parent or guardian.

Can I ask the driver to make an unscheduled stop along the way?

No. Drivers are not permitted to make additional stops.

What should I do if the Transportation Service is running 15 minutes late?

Please call the Transportation Service Office to ensure that the vehicle is on its way.

Where should I wait for my ride home after my appointment?

Please wait in the waiting room after your appointment and your driver will come get you.

Passenger Rules:

Waiting for Your Ride

  • Transportation may arrive 15 minutes before or after the time scheduled
  • The driver will wait at the pick-up location for 5 minutes, so please be prepared and punctual.
  • Under no circumstances will a driver enter a passenger’s private residence.
  • The driver may help passengers on and off the vehicle and may also help them secure their wheelchair.
  • LIHC Transportation Services abide by the IHS (Indian Health Service) Indian Health Manual, Chapter 16, and Transportation Policy which restricts transportation of patients to agencies or locations other than IHS Facilities or IHS referral sites.

Aboard the Vehicle

  • No eating or drinking
  • No smoking
  • No profanity
  • No weapons
  • No disruptive behavior
  • No drugs or alcohol

Cancelling your Ride

Should you need to cancel or reschedule your ride, please call the Transportation Service Office at least 48 hours in advance. While your clinic appointments may change, it is your responsibility to reschedule your ride accordingly.
